As a result of social distancing practices, in order to obtain a birth certificate, customers must print an application for certified copies from the website through the health department . Customers may also request a form by email or by telephone. Customers should fill out the application and submit it, along with the $24 fee, via mail. For those customers who are local, they may utilize our drop-box for applications and payments as well. Checks and money orders should be made payable to Kent City Health Department. Birth Records can also be obtained from any other health department in the state of Ohio that holds vital records, though we recommend calling ahead to any health department you choose.
Death Certificates can be filed through our dedicated email address (firstname.lastname@example.org) or can be sent via US mail. Funeral homes must contact the Kent City Health Department by phone for exceptions. Funeral homes may order death certificates by credit card by supplying credit card information during the electronic filing process or calling our office directly.
